About the Test

Test Facts

Important: After submitting the order form, you will receive a Sample ID number via email. Please remember to write your Sample ID# and Date of Birth in ink on the outside of the test tube.

DO NOT write your name to keep your test confidential.​

  • A medical professional will review all submitted forms and order a test for all eligible patients.​

  • The information collected on the form below is solely used to ship a saliva collection kit to you. Nothing will be shared outside of this purpose.

  • The test is completely de-identified -- no one will know your status except you and the medical professional who reviews your results.

  • This test is conducted by a company certified to CLIA/CAP standards, ensuring that results are reliable and the test itself meets or exceeds industry standards for clinical laboratory testing.

  • This test is also HIPAA compliant, ensuring security around your results.

  • The test is currently free to residents of the United States thanks to funding from the PML Foundation. If you are interested in getting tested outside of the US, please email us at info@pmlrisktest.org.
